Get ready for your first appointment

There’s a lot to do to get ready for your first appointment with your clinician, but we’re committed to making it as easy as possible:

Bring the basics:

  • Your ID
  • Your insurance card
  • A current list of medications and/or supplements

Fill out your new patient forms.

You may also want to review our patient policies to understand our no-show policy, split billing, and patient code of conduct.
