Health Information Exchanges
Tryon Medical Partners participates in the following Health Information Exchanges (HIEs). Tryon shares your PHI with HIEs and may use these HIEs to access your PHI to assist us in providing health care to you.
North Carolina HealthConnex HIE (“HealthConnex”) | Tryon shares your PHI with HealthConnex through the North Carolina Information Exchange Authority
- If you do not want your PHI accessible to other HealthConnex participants, you must opt out by submitting a form directly to HealthConnex. The opt out form may be downloaded directly from the NC Health Information Authority Website: https://hiea.nc.gov/patients/your-choices. Information explaining the benefits of the HealthConnex HIE and instructions on how to opt out are available on this website as well. Your opt out will also not affect our obligation to disclose your PHI to HealthConnex when you receive hospital services that are paid for by Medicaid or other NC State funded resources.
CareConnect HIE (“CareConnect”) | Tryon shares your PHI with CareConnect through Atrium Health.
- If you do not want your PHI input into CareConnect, you must opt out by submitting a form directly to CareConnect. Information about opting out is available directly on the CareConnect website: https://www.atriumhealthcareconnect.org/providers/what-are-my-patients-choices
Zus Network (“Zus”) | Tryon shares your PHI with the Zus Network for purposes of coordinating and providing safe, convenient, integrated care to you whenever you see a provider participating in Zus’s network.
- If you do not want your PHI accessible to other Zus participants, you must opt out by submitting a form directly to Zus. The opt out form may be accessed directly from the Zus Health Website: https://zushealth.com/zus-request-to-opt-out-form/. Information explaining Zus and how it works to protect your privacy can also be found on this website.
Even if you opt out of the HIEs, we may use your PHI available from any of these HIEs in order to provide emergency care to you or for public health or research purposes authorized by law.
Research
Tryon Medical Partners works with Javara Research to offer clinical trials to our patients. To the extent permitted by law, Tryon shares limited patient demographic information with Javara to allow them to prepare for clinical trials with our Principal Investigators and at our sites. Javara may contact you on our behalf to inform you of potential research studies. If you do not wish to have your information shared with Javara, please use the “Request for Restriction of Use or Disclosure of Protected Health Information (PHI)” form below to opt out.
Opting Out
Tryon respects your right to choose how and with whom your information is shared. If you wish to restrict how we share your information with other individuals or entities, please submit a completed form to our Privacy Compliance Office.
