How to lower triglycerides: what you need to know about this important health marker

When you receive your annual lipid panel blood test results, you might focus primarily on your overall cholesterol numbers, but there’s another crucial measurement that deserves your attention: triglycerides. These blood fats play an important role in your cardiovascular health and can serve as an early warning system for conditions like diabetes, metabolic syndrome, and heart disease. Understanding what your numbers mean and how to lower triglycerides to a healthy range can significantly impact your long-term health.

What are triglycerides?

“Triglycerides are a type of lipid that we use for energy in our bodies, stored in fat cells. They are released when we need that energy,” NP Eaton explains.

Unlike cholesterol, which your body uses to build cell membranes and produce hormones, triglycerides function primarily as an energy storage system. When you eat, your body converts calories it doesn’t immediately need into triglycerides and stores them in fat cells. Between meals, hormones trigger the release of these triglycerides to provide energy for your body’s functions.

This system works efficiently when triglyceride levels remain balanced. Your body stores just enough energy for normal function and releases it as needed throughout the day. However, when you consistently consume more calories than you burn, particularly from certain types of foods, triglyceride levels can climb higher than healthy ranges, creating health risks beyond simple energy storage.

Understanding triglycerides as an energy management system helps explain why diet, exercise, and weight management play such crucial roles in keeping these levels optimal. Everything you eat that isn’t immediately used for energy has the potential to become triglycerides, making your daily food choices directly relevant to your lipid panel results.


Why do healthcare professionals test triglyceride levels?

Triglyceride testing provides valuable information that extends well beyond a single number on your lab report. “We get a lipid blood test once a year, which helps us break down the components of the lipid and identify underlying medical conditions,” NP Eaton says. “This helps us identify risk level for conditions like insulin resistance, type two diabetes, and is associated with cholesterol that can increase risk of cardiovascular disease.” 

The lipid panel blood test typically measures total cholesterol, LDL cholesterol (often called “bad” cholesterol), HDL cholesterol (often called “good” cholesterol), and triglycerides. Together, these numbers provide your healthcare clinician with essential information about your cardiovascular risk and metabolic health.

Testing frequency depends on your individual circumstances. Healthy patients are typically tested annually for prevention and establishing a baseline, but if patients are on medication like statins, they may get tested more frequently. 

If you’re managing high cholesterol with medication or have other cardiovascular risk factors, your clinician may order lipid panels more often to monitor treatment effectiveness and adjust your care plan as needed. For patients already diagnosed with conditions like diabetes or heart disease, more frequent testing helps track whether interventions are working and allows for timely adjustments to medication or lifestyle recommendations.


What is the ideal triglyceride level?

Understanding what your triglyceride numbers mean helps you gauge your cardiovascular health and metabolic function. The ideal triglyceride level in a lipid blood test is less than 150. This threshold represents the target for optimal health, though levels can be further categorized based on severity when elevated.

  • Normal: Less than 150 mg/dL 
  • Borderline high: 150-199 mg/dL 
  • High: 200-499 mg/dL 
  • Very high: 500 mg/dL or above

The primary importance of maintaining a triglyceride level below 150 is that it will reduce your cardiovascular risk. While elevated triglycerides don’t directly cause heart attacks the way blocked arteries do, they contribute to conditions that increase cardiovascular risk over time.

Beyond this indirect cardiovascular impact, triglycerides “can directly cause conditions like pancreatitis and fatty liver disease,” shares NP Eaton. Pancreatitis (inflammation of the pancreas) becomes a serious risk when triglyceride levels climb extremely high, typically above 500 mg/dL. This acute condition requires immediate medical attention and can be life-threatening.

High triglycerides may also signal broader metabolic problems. “If levels are high, they often signify a diet that can cause metabolic syndrome, and excess alcohol use, as well as obesity and type 2 diabetes,” NP Eaton notes. This clustering of conditions (metabolic syndrome) dramatically increases cardiovascular disease risk and requires comprehensive intervention.


What causes high triglycerides?

Understanding the causes of high triglycerides empowers you to take targeted action to lower them. Multiple factors can elevate triglyceride levels, many of which relate to lifestyle choices you can modify.

  • Dietary factors play a primary role in triglyceride elevation. When NP Eaton mentions that high triglyceride levels often correlate with “a diet that can cause metabolic syndrome,” she’s referring to eating patterns high in refined carbohydrates, added sugars, and overall excess calories. Foods that quickly convert to sugar in your bloodstream, like white bread, pasta, sugary beverages, candy, and baked goods, particularly drive triglyceride production.
  • Alcohol consumption significantly impacts triglyceride levels. “Excess alcohol use” often appears among patients with elevated triglycerides, NP Eaton notes. Even moderate drinking can raise triglycerides in susceptible individuals, while heavy drinking almost invariably causes elevation. Alcohol provides empty calories that your liver readily converts to triglycerides, and it also impairs your liver’s ability to clear triglycerides from your bloodstream.
  • Obesity and excess weight create a self-reinforcing cycle with triglycerides. Carrying extra weight, particularly around your midsection, both results from and contributes to elevated triglycerides. Excess body fat produces inflammatory substances that interfere with normal triglyceride metabolism, while high triglyceride levels promote additional fat storage.
  • Type 2 diabetes and insulin resistance frequently accompany high triglycerides. These conditions impair your body’s ability to properly process and store energy, leading to triglyceride accumulation in the bloodstream. High triglycerides can also worsen insulin resistance, creating a problematic cycle that increases diabetes risk.
  • Physical inactivity allows triglycerides to remain elevated rather than being burned for energy. Regular physical activity helps your body use stored triglycerides for fuel, naturally lowering levels over time.
  • Genetic factors affect some people’s triglyceride metabolism, making them more prone to elevation even with relatively healthy lifestyles. However, lifestyle modifications still provide significant benefits even for those with a genetic predisposition.
  • Certain medications, including some blood pressure drugs, steroids, and estrogen preparations, can raise triglyceride levels as a side effect. If you’re taking medications and notice elevated triglycerides, discuss this with your healthcare clinician rather than stopping medications on your own.

What are the symptoms of high triglycerides?

Unlike many health conditions, high triglycerides typically don’t produce noticeable symptoms until levels become extremely elevated or complications develop. This silent nature makes regular lipid panel blood tests essential for detection.

  • Most people with moderately elevated triglycerides (150-499 mg/dL) feel completely normal. They have no physical sensations indicating a problem, no obvious warning signs, and no symptoms that would prompt them to seek medical care. This is why healthcare clinicians emphasize annual screening: the only way to catch elevated triglycerides at this stage is through blood testing.
  • When triglycerides climb to very high levels (500 mg/dL or above), some people may develop visible symptoms. Extremely high triglycerides can cause small fatty deposits called xanthomas to appear on the skin, particularly on the elbows, knees, hands, or feet. Some people develop a whitish ring around the cornea of the eye.
  • The most concerning symptom of severely elevated triglycerides is acute pancreatitis (sudden inflammation of the pancreas). This presents with severe abdominal pain, nausea, vomiting, and requires emergency medical care. However, pancreatitis only occurs when triglycerides reach extremely high levels, typically well above 500 mg/dL.

The absence of symptoms underscores why triglycerides deserve attention even when you feel perfectly healthy. “Even for younger people, it is very important to do the preventive screenings to identify problems early and prevent complications,” NP Eaton emphasizes. Waiting until symptoms appear means missing the opportunity for early intervention when lifestyle changes alone might normalize levels.


How to lower triglycerides: treatment options and lifestyle changes

The good news about elevated triglycerides is that they respond well to intervention, particularly lifestyle modifications. Treatment approaches depend on how high your levels are and what other health conditions you may have.

Lifestyle adjustments form the foundation of triglyceride management for most people. “If focusing on lifestyle adjustment: focus on maintaining a healthy weight and diet (reduce sugar, reduce refined carbohydrates, increase fiber intake, limit alcohol),” NP Eaton advises.

Specific dietary changes that lower triglycerides include:

  • Reducing or eliminating added sugars and sugar-sweetened beverages
  • Choosing whole grains over refined carbohydrates
  • Increasing fiber intake through vegetables, fruits, and whole grains
  • Limiting or avoiding alcohol consumption
  • Incorporating healthy fats from fish, nuts, and olive oil
  • Reducing portion sizes to achieve and maintain a healthy weight

Physical activity helps burn triglycerides for energy while improving insulin sensitivity and supporting weight management. Even moderate exercise like brisk walking for 30 minutes most days can significantly impact triglyceride levels.

Medications for triglycerides are “reserved for moderate to severe patients,” states NP Eaton. When lifestyle changes prove insufficient, or levels are dangerously high, several medication options exist. Traditional medications include fibrates and high-dose omega-3 fatty acids specifically formulated for triglyceride reduction.

“Some of the medication that patients might be more interested in at this point is GLP-1s, which help patients lose weight along with diet, exercise, and reduction of alcohol consumption, starchy carbohydrates, etc. These will help indirectly to lower triglycerides,” NP Eaton notes. These newer medications, originally developed for diabetes management, have shown remarkable effectiveness for weight loss and metabolic improvement, which in turn lowers triglycerides.

Unlike cholesterol, where very low levels can occasionally cause problems, “there is no too low level for triglycerides,” NP Eaton clarifies. “You do want enough cholesterol in your body to support cellular function, but triglycerides function differently. Your body efficiently produces the triglycerides it needs from the food you eat, so having very low fasting triglyceride levels doesn’t create health problems.”


What’s the difference between triglycerides and cholesterol?

Both triglycerides and cholesterol are lipids (fats) in your blood, and both appear on a lipid panel blood test, but they serve different functions and pose different health risks.

Cholesterol is a waxy substance your body uses to build cell membranes, produce hormones (including estrogen, testosterone, and cortisol), and create vitamin D. Your liver produces most of your cholesterol, with dietary sources providing the remainder. Cholesterol travels through your bloodstream in different forms: LDL (low-density lipoprotein) carries cholesterol to cells where it’s needed, while HDL (high-density lipoprotein) carries excess cholesterol back to the liver for disposal.

Triglycerides, as previously explained, function primarily as energy storage. Your body packages excess calories (particularly from carbohydrates and fats) into triglycerides for later use.

The health implications differ as well. High LDL cholesterol directly contributes to atherosclerosis: the buildup of plaque in arteries that leads to heart attacks and strokes. This creates a direct cardiovascular risk. Elevated triglycerides, by contrast, create what NP Eaton describes as “more of an indirect risk factor” for cardiovascular disease. They contribute to metabolic syndrome, insulin resistance, and inflammation that increase heart disease risk over time, but don’t typically cause plaque buildup in arteries the way LDL cholesterol does.

Treatment approaches also differ. Statins (the most common cholesterol medications) primarily target LDL cholesterol and don’t significantly lower triglycerides. Conversely, medications specifically for triglycerides don’t dramatically impact LDL cholesterol. This is why your lipid panel measures both separately and why treatment plans often address each independently.

“We don’t focus on it as much as the LDL portion of the lipid blood test, but it’s important, and we shouldn’t skip over it,” NP Eaton cautions. While LDL cholesterol often receives more attention in cardiovascular discussions, triglycerides provide crucial information about metabolic health and disease risk that LDL alone cannot reveal.
