How to stay connected to your care team
There's a lot to do to get ready for your first appointment, but we're committed to making it as easy as possible.

1. Arrive early and bring the basics.
Be sure to arrive 30 minutes before your appointment time, and don’t forget three important items:
1. Your ID
2. Your insurance card
3. A current list of medications and/or supplements
2. Complete your paperwork.
Download and complete your new patient forms.
You may also want to review our patient policies to understand our no-show policy, split billing, and patient code of conduct.
3. Fast for 8 hours before bloodwork.
